Any person desiring to construct a driveway entrance onto a lot or parcel of land from an existing Town road or a road which is to be dedicated to the Town shall obtain a permit from the Town Highway Superintendent for the entrance prior to commencement of such construction. The construction performed shall be in accordance with provisions of a plan approved by the Town Planning Board and any other conditions imposed by the Town Highway Superintendent.
A. 
Curb cuts.
(1) 
Curb cuts for driveway entrances shall be made in accordance with Figure 12 in cases where concrete or asphalt concrete curbs must be traversed.[1]

(2) 
In subdivisions where roads are approved under these Town highway specifications, driveway entrances shall be formed at the time of curb construction.
B. 
Ditch crossings. Where driveway entrances involve ditch crossings, the configuration of the driveway entrances shall be similar to that shown in Figure 12,[2] and the ditch shall be replaced with piping of adequate size and strength to accept the flows and vehicle loadings anticipated. The piping shall extend a minimum of 15 feet to either side of the center line of the proposed entrance.

C. 
Grades. All driveways with a grade of 3% or greater shall be paved with three inches' minimum thickness of asphalt concrete and as required by the Town Superintendent of Highways or the Town Engineer. In no instance shall the grade of a proposed driveway exceed 14%.
