§ 119-1 Town Clerk.

Pursuant to the provisions of Section 73 of Chapter 329 of the Acts of 1980,[1] the following fees are established for the office of Town Clerk:

	(1)	For filing and indexing assignment for the benefit of creditors: $5.
	(11)	For entering amendment of a record of the birth of an illegitimate child subsequently legitimized: $5.
	(12)	For correcting errors in a record of birth: $10.
	(13)	For furnishing a certificate of birth: $10.
	(13A)	For furnishing an abstract copy of a record of birth: $2.
	(14)	For entering delayed record of birth: $5.
	(20)	For filing a certificate of a person conducting business under any title other than his real name: $50. [Amended 11-18-2013 RTM by Art. 14]
	(21)	For filing by a person conducting business under any title other than his real name of a statement of change of his residence or of his discontinuance, retirement or withdrawal from or of a change of location of such business: $10.
	(22)	For furnishing a certified copy of a certificate of a person conducting business under any title other than his real name or a statement by such person of his discontinuance, retirement or withdrawal from such business: $3.
	(24)	For recording the name and address, the date and the number of the certificate issued to a person registered for the practice of podiatry in the commonwealth: $10.
	(29)	For correcting errors in a record of death: $10.
	(30)	For furnishing a certificate of death: $10.

	(58)	For recording the name of the owner of a certificate of registration as a physician or osteopath in the commonwealth: $10.
	(62)	For recording an order granting locations of poles, piers, abutments or conduits, alterations or transfers thereof and increase in number of wires and cables or attachments under the provisions of MGL c. 166, § 22: flat rate of $50 and an additional fee of $5 and $3.50 additional for each street or way included in such order.
	(66)	For examining records or papers relating to a birth, marriage or death upon the application of any person: the actual expense thereof, but not less than $5.
	(67)	For copying any manuscript or record pertaining to a birth, marriage or death: $3.
	(69)	For receiving and filing of a complete inventory of all items to be included in a closing-out sale, etc.: $2.
	(75)	For filing a copy of a written instrument or declaration of trust by the trustees of an association or trust or any amendment thereof as provided by MGL c. 182, § 2: $10.
	(79)	For recording any other documents: $10 per page.

§ 119-1.1 Town Clerk — Registration of underground storage tanks.

Pursuant to MGL c. 148, § 13, the Town Clerk shall charge the following fees for the annual registration of underground storage tanks:

§ 119-2 Other boards and agencies.

The following fees are maximum fees which may be charged by the appropriate board or licensing agency:

A. Sealer of Weights and Measures.

(1) Liquid capacity measure of capacity of more than one gallon and measures on pumps: $40 each.

(2) All other weights and measures: $25 each.

B. Other fees:

(1) License for automatic amusement devices: $100.

(2) License for innholders and restaurants: $50.

(3) License for fortune-tellers: $50.

(4) Transient vendor license: $50 plus $25 per employee.

(5) Common victualer license: $25.

(6) Innholder license: $50.

(7) Video machine license: $100.

(8) Entertainment license: $50 per device.

§ 119-3 Building permit fee schedule.

P. Tank installation fee (above or under ground): $50.
