The name of the proposed Authority is "Mon Valley Health and Welfare Authority."
The Articles of Incorporation of the Authority are as follows:

Articles of Incorporation
To the Secretary of the Commonwealth of Pennsylvania:
In compliance with the Municipality Authorities Act of 1945, its supplements and amendments, the City of Monessen, Westmoreland County, a municipality of the Commonwealth of Pennsylvania, pursuant to an ordinance duly adopted by the Council of the City of Monessen, signifying their desire and intention to form an Authority, hereby certify:
1.
The name of the authority shall be "Mon Valley Health and Welfare Authority."
2.
This Authority is formed under the Municipality Authorities Act of 1945, its supplements and amendments.
3.
The City of Monessen has previously organized the following Authorities: (a) Parking Authority; (b) Civic Center Authority; (c) Municipal Authority (inactive); (d) Mon Valley Sewage Authority (this Authority is a joint Authority formed by the City of Monessen and the Borough of Donora). None of the above Authorities organized under the Municipality Authorities Act of 1945, its supplements and amendments, is in existence for the same purposes.

The purpose of the Authority is to provide community health services of all kinds and types for the residents of the City of Monessen and the residents of all other municipalities within reasonable service distance of the City of Monessen, including but not limited to Allenport, Belle Vernon, Black Diamond, Carroll Township, Charleroi, Courtney, Donora, Dunlevy, Elco, Fallowfield Township, Fayette City, Long Branch, Monongahela, New Eagle, North Belle Vernon, North Charleroi, Roscoe, Rostraver Township, Speers, Stockdale, Twilight, Washington.
5.
The incorporating municipality is: THE CITY OF MONESSEN, WESTMORELAND COUNTY, PENNSYLVANIA.

The names and addresses of the Mayor and members of Council of the City of Monessen, such persons being the municipal authorities of the municipality, are as follows:[1]

The names and addresses of the first board of the Authority to be formed are hereby listed:[2]
In witness whereof the City of Monessen has executed these Articles, by its proper officers thereunto duly authorized, and, under its municipal seal, this 20th day of April, A.D. 1965.

The following persons are hereby appointed to the Board of Authority, with the following terms of office:

Robert B. Kramer, Treasurer: three years.

Normal Cotter: two years
E. 
Dr. J. Dudas, Jr.: two years.

C.A. Breuer: one year.
G. 
Edwin E. Caspell: one year.
The total annual salary to be paid the Authority Board members shall be set forth from time to time by resolution.

The Articles of Incorporation of the Authority, in substantially the form set forth in § 5-13 of this article, shall be executed on behalf of the City of Monessen by the Mayor, and under its municipal seal, attested by the City Clerk of the City of Monessen, and such officers are hereby authorized, empowered and directed to do all things necessary and appropriate to effect and establish said Authority in conformity with the Municipality Authorities Act of 1945, its supplements and amendments.[1]
