Whenever the health officer determines that a violation of any provision of this chapter has occurred, the Health Department shall give notice to the person responsible for such violation. The notice shall:
A. 
Be in writing;
B. 
Include a statement of the reasons for issuance of the notice;
C. 
Allow reasonable time, as determined by the health officer, for performance of any act it requires;
D. 
Be served upon the person responsible for the violation(s) by certified mail, return receipt requested, to his/her last known address as furnished by the permit applicant to the Health Department or by any other method of service of legal process as authorized by laws of this state; and
E. 
Contain an outline of remedial action which is required to effect compliance with this chapter.
