The certification required by this article shall be in the form of a sworn statement signed by an officer or director of the agency or organization and shall be submitted with any voucher for payment made to the County of Suffolk.
Failure to certify employees and volunteers shall result in a first-time warning; a second offense will result in a fifty-percent loss of funding for that contracted program; and a third offense shall result in a loss of the contract for a minimum of two years.
