The rules and regulations attached hereto as Exhibit A and incorporated herein are hereby approved and adopted.
The disclosure statement, substantially in the form set forth in Schedule 1 of the said rules and regulations, is hereby approved and adopted.
The form of request for time to provide information, substantially in the form set forth in Schedule 2 of the said rules and regulations, is hereby approved and adopted.
The form of petition for appeal and refund, substantially in the form set forth in Schedule 3 of the said rules and regulations, is hereby approved and adopted.
The administrative appeal procedures set forth in the rules and regulations and the administrative appeal procedures application to petitions for appeals and refunds set forth in Schedule 4 of the said rules and regulations are hereby approved and adopted.
The form of initial refund claim, substantially in the form set forth in Schedule 5 of the said rules and regulations, is hereby approved and adopted.
The procedures regarding initial refund claims set forth in the rules and regulations and the initial refund claim form procedures set forth in Schedule 6 of the said rules and regulations are hereby approved and adopted.
The Town Council hereby determines that administrative appeal procedures relating to petitions for appeal and refund submitted by taxpayers in connection with the assessment, determination or refund of an eligible tax under the Local Taxpayers Bill of Rights[1] shall be undertaken by the Town Council of the Borough in executive session, as authorized by the LTBR.
