The Town Board of the Town of Addison establishes the following standards:
A. 
The applicant shall submit a completed operator's license application to the Town Clerk.
B. 
The applicant shall authorize the release of personal criminal information to the Town of Addison.
C. 
The applicant shall be enrolled in a responsible beverage service training course.
D. 
The Town Clerk is authorized to issue the provisional operator's license.
E. 
A provisional operator's license may not be issued to any person who has been denied a license under § 125.17(1), Wis. Stats., by the Town Board of the Town of Addison.
F. 
The fee for a provisional operator's license[1] shall be as set by the Town Board.[2]

G. 
The provisional operator's license shall expire 60 days after its issuance or when an operator's license under § 125.17(1), Wis. Stats., is issued to the holder, whichever is sooner.
H. 
The provisional operator's license shall be revoked if it is discovered that the holder of the license made a false statement on the application or if the personal criminal information is determined to be unfavorable for a person holding an operator's license.
