A. 
Water and sewer rates of the combined Warsaw water and sewerage system shall be as follows:

Water base fee
$7.17 per month
All water usage
$4.95 per 1,000 gallons or part thereof
Sewer base fee
$12.74 per month
All sewer usage
$5.33 per 1,000 gallons or part thereof
B. 
The process the Utility Department shall follow for billing and collection of amounts due for service is as follows:
1. 
All bills are due and payable on the first (1st) of the month in which billed.
2. 
Bills become delinquent twenty (20) days after the first (1st) day of the month.
3. 
Any water user not paying their current bill within twenty (20) days after the first (1st) day of the month shall be assessed a ten percent (10%) penalty on the entire utility bill.
4. 
Upon failure to pay the bill and penalty within ten (10) days, utility service shall be subject to disconnection.

C. 
Any water user for whom service has been terminated for non-payment of bill and penalty shall not have water restored until the bill, penalty, restoration fee of thirty-five dollars ($35.00), if during business hours, or one hundred dollar ($100.00) if after hours, and a deposit of one hundred dollar ($100.00) has been paid.

D. 
Any unauthorized turn-on of water service after a utility service has been terminated for non-payment of bill and penalty shall be assessed a tampering charge of seventy-five dollars ($75.00) and meter shall be pulled. Service shall not be restored until delinquent bill, penalty, restoration fee, tampering charge and one hundred dollar ($100.00) deposit has been paid.
E. 
A water deposit of one hundred dollars ($100.00) shall be required for all users.
F. 
Any non-residential property consisting of multiple units and having the facilities for multiple units shall be charged by number of units for minimum water and sewer services. Any usage over minimum service charges shall be calculated at current per gallon rates on total consumption.
G. 
Unless set aside by a majority vote of the Board of Aldermen, no property not annexed within the City limits of Warsaw shall obtain utility services.
H. 
All owners of the property served shall be required to install an accessible shut-off valve on the premises. Should a service order be issued for a shutoff of service for repairs or other issue, an accessible shut-off valve shall be required to be installed before the service can be turned back on.
I. 
All new connection services shall require installation of a backflow preventor.
J. 
A twenty-five dollars ($25.00) service charge for a returned check/ACH or any other form of payment returned to the City.

A. 
Water customers requesting an adjustment in their water bill as a result of a plumbing leak must contact the City Clerk and request a water leak/pool fill adjustment form. The plumbing leak adjustment request must provide all of the information set forth in the water leak/pool fill adjustment form. The water leak/pool fill adjustment form must be completed in full prior to its consideration by the City.
B. 
Water customers requesting an adjustment in their water bill as a result of filling a pool must contact the City Clerk and request a water leak/pool fill adjustment form. The water leak/pool fill adjustment form must be completed in full prior to its consideration by the City. Pool fill adjustment requests will be granted only once per calendar year.
C. 
Upon completion of the water leak/pool fill adjustment form, said form shall be returned to the City for its consideration and determination as to whether an adjustment will be granted.
D. 
The determination as to whether to grant the water leak/pool fill adjustment request shall be made by the Warsaw City Clerk.
E. 
Swimming Pool Credit. This credit will not be provided more than once annually. To receive the credit, a completed utility bill adjustment request form must be submitted. The utility account will be credited for wastewater usage for the actual gallons of water used to fill the pool.
F. 
Plumbing Leak Credit.
1. 
Plumbing leaks that have been repaired will be credited at the City's current rate. This credit will be provided not more than twice annually, for no more than two (2) months (billing cycles) each time. To receive the credit, a completed utility bill adjustment request form must be submitted along with proper documentation of the repair (such as a receipt from a plumber, or before and after photos, etc.)

The utility account will be credited for wastewater usage above the average use for the prior twelve (12) months of service for documented repair of plumbing leaks. If the leak was in the house and the leaked water went down the drain (for instance, by a faulty toilet flapper valve), no credit will be given for wastewater.
G. 
Payments. Once the form is received by the utility office, payment on the abnormal bill will be suspended above the amount of the average of the previous three (3) months until a determination of credit is issued. The customer will be notified by phone or e-mail within ten (10) business days if the credit was approved or denied. Written notification of approval or denial will be mailed to the customer. The remaining balance would include any adjustments given. Payment arrangements may be given by splitting the remaining bill in three (3) equal installments due by the 20th of each month.

In addition to the amounts due for water/sewer as set forth in Section 700.030, the City shall charge an online convenience fee for those customers using online credit card services; and
B. 
The online convenience fee charged to customers shall be the same as the fee charged to the City and shall, in essence, be a reimbursement to the City.

A permit, inspection and connection fee to the City's water system shall be in the amount of five hundred dollars ($500.00) for a residential or commercial building permit and one thousand dollars ($1,000.00) for an industrial permit.
