Borough of Rockleigh, NJ Chapter BH13 Fees

§ BH13-1 FEES ESTABLISHED.

There are hereby established the following fees:

a. Vital Statistics.

Vital Statistics
1.	Certified copies of Birth Certificate	$20.00*
2.	Certified copies of Death Certificate	$20.00*
	Note: Veterans entitled to one (1) death certificate at no charge.
3.	Certified copies of Marriage License:	$20.00*
4.	Certified copies of Domestic Partnership Certificate or Civil Union Certificate	$20.00*
5.	Request for correction to NJ Vital Record	$10.00

*Additional copies of Certified Copies shall be $10.00 for each additional copy.

b. Environmental Health.

Environmental Health
1.	Septic Permits.
	(a) New Construction/Review	$600.00
	(b) Alteration/Repair	$250.00
	(c) Reinspection	$100.00
2.	Wells.
	(a) New Well	$200.00
	(b) Alteration/Repair	$50.00
	(c) Well Pump Permit	$50.00
	(d) Reinspection	$75.00
3.	Public Recreational Bathing.
	(a) Whirlpools and Hot Tubs (per unit)	$100.00
	(b) Swimming Pool (For Profit)	$250.00
	(c) Wading Pool	$50.00
	(d) Reinspection	$50.00
4.	Laboratories.
	(a) Inspection	$500.00

c. Food Safety. Annual retail food establishment licenses and associated fees shall be defined as follows:

Food Safety
1.	CLASS I: Food and Drink. Any retail or wholesale food establishment including those defined in Chapter 24 of the New Jersey State Sanitary Code in which food or drink is prepared, served, handled or provided for the public with or without charge.
	(a) Non-Seating	$100.00
	(b) Prepackaged only	$50.00
	(c) 1-50 seats	$170.00
	(d) 51-100 seats	$230.00
	(e) 101+ seats	$375.00
	(f) Nursing Home, Long-Term Care and Assisted Living Facilities	$425.00
2.	CLASS III: School. Any food establishment defined as educational	$100.00
3.	CLASS IV: Mobile Food and Drink shall include any motor vehicle or any vehicle cart, or bicycle of any kind including hand carried portable containers in which food or drink is transported, stored or prepared at temporary locations	$100.00 per vendor for one event per month per calendar year; or $20.00 per vendor per event.
4.	CLASS V: Temporary Retail Food Establishment. Any food establishment as defined in Class I which operates at a fixed location for a temporary period of time not to exceed seven (7) days in connection with a fair, carnival, circus, public exhibition or similar organizational meetings, mobile retail food establishments as well as agricultural markets	$100.00
5.	CLASS VI: Vending Machine. Any self-service device which, upon insertion of a coin, paper currency, token card, key or by any other means, dispenses unit servings of food, either in bulk or in packages without the necessity of replenishing the device between each vending operation.
	(a) Prepackaged only — coffee/tea, sandwiches, candy, ice cream cookies	$30.00 per
	(b) All others	$30.00

§ BH13-2 COLLECTION.

The Board of Health shall collect certain fees and deposit them in accordance with the established policy of the Borough of Rockleigh.

§ BH13-3 VIOLATIONS AND PENALTIES.

Any person or persons, firm or corporation violating any of the provisions of or order promulgated under this chapter shall, upon conviction thereof, pay a penalty of not less than fifty ($50.00) dollars nor more than five hundred ($500.00) dollars. Each day a particular violation continues shall constitute a separate offense.
