The Notice of Tort Claim form is hereby adopted by the Township
of West Deptford as the official form for all persons making any claims
against the Township of West Deptford. All persons making any claims
shall be required to complete the form adopted as a condition of compliance
with the notice requirements of the New Jersey Tort Claims Act, N.J.S.A.
59:8-1 et seq.[1]
