§ BH10-1 FEES ESTABLISHED.

There are hereby established the following fees:

a. Vital Statistics.

1. Birth Certificate: $10.

2. Death Certificate: $10.

Note: Veterans entitled to one death certificate at no charge.

3. Marriage Certificate: $10.

4. Domestic Partnership Certificate: $10.

5. Marriage License: $28.

6. Domestic Partnership: $28.

7. Burial Permit: $5.

b. Personal Health.

1. Coronary risk profile to include SMAC blood test: $15-$25/test.

2. Influenza vaccinations: $15/vaccination.

3. Female Pap: $20-25/test.

4. Male Prostate: $20-25/test.

5. Adult tetanus vaccination: $25/vaccination.

c. Environmental Health.

1. Septic Permits.

(a) New Construction: $150.

(b) Alteration/Repair: $100.

(c) Reinspection: $25.

2. Wells.

(a) New Well: $350.

(b) Alteration/Repair: $25.

(c) Well Pump Permit: $35.

(d) Reinspection: $25.

3. Public Recreational Bathing.

(a) Bathing Beach: $125.

(b) Hot Tub/Spa: $125.

(c) Swimming Pool: $125.

(d) Wading Pool: $125.

d. Food Safety.

Annual Retail Food Establishment Licenses and associated fees shall be defined as follows:

e. Body Art.

1. Tattooing and Permanent Cosmetics.

(a) Initial: $200.

(b) Renewal: $100.

2. Body Piercing.

(a) Initial: $100.

(b) Renewal: $50.

§ BH10-2 COLLECTION.

The Health Department shall collect certain fees and deposit them in accordance with the established policy of the Borough of Bloomingdale.

§ BH10-3 VIOLATIONS AND PENALTIES.

[Ord. No. 2005-BOH-01 § 3]

Any person or persons, firm or corporation violating any of the provisions of or order promulgated under this chapter shall, upon conviction thereof, pay a penalty of not less than $100 nor more than $1,000. Each day a particular violation continues shall constitute a separate offense.
