Each employee receiving wages or regular compensation from the Township shall be required to complete daily time reports, on forms or cards to be prepared by the Administrator. They shall be required to observe and follow the procedure herein stated:
A. 
Each department head and each employee shall make out and maintain his own daily time report, completing such information on such report as shall be directed by the Township Administrator. For each report made out by an employee, the information contained therein must be certified to by the department head.
B. 
At the end of each week, the department head shall be responsible to collect and receive from the employees under his head all of the daily time reports prepared by them, shall examine them, and shall certify them as to accuracy or inaccuracies.
C. 
On the Monday immediately following, the department head shall file all of the daily time reports received by him with the Township Treasurer.
D. 
For daily time reports prepared by and for the employees of the Township Division of Streets and Roads in the Department of Public Works and Engineering, the following additional information shall be noted on the reports: a daily analysis of work performed by the employee, with certification by the department head.
A. 
Any employee who is injured in the performance of his duties shall immediately report the accident to his superior. Whenever an accident or any incident shall occur involving damage to Township equipment or property, damage to property of others, personal injury to any Township official or employee, or personal injury to others, the following written reports shall be filed:
(1) 
A detailed report to be completed and filed by the officer or employee involved in the accident or incident, to be filed with his department head, within 24 hours after the alleged accident, with sufficient copies of the report for the Township Administrator, department head, and for the Township insurance carrier. Upon receipt of the reports and copies, the department head shall forthwith forward the report and copies to the Township Administrator who shall, upon receipt of same, transmit the copies thereof to all necessary persons. The report so filed shall contain the following information: the date, time, and place of the accident; the names and addresses of the parties involved; a description of the damages or injuries sustained; the names and addresses of witnesses. In the event the officer or employee is unable to file the report within the period above stated because of injury sustained, then such report should be sought for by the department head from any other employee available and who might have the information sought for. The report of the accident shall be secured from the employee involved as soon as he is able to make such report.
B. 
The department head, following receipt of such report, and not later than one week from the date of the accident or occurrence, shall make and complete his investigation of the same, and shall within such period submit his written report of his findings with sufficient copies thereof for the Administrator and Township insurance carrier. The report and copies shall be filed with the Township Administrator who shall, upon receipt of the same, transmit the copies thereof to the necessary persons. The report so filed shall contain the following information:
(1) 
Detailed statement of investigation made.
(2) 
Names and addresses of parties interviewed.
(3) 
Responsibility of Township employees involved.
(4) 
Opinion as to Township responsibility.
(5) 
Estimate of damages to Township equipment or property, with repair estimates or costs.
(6) 
Recommendation as to repair or replacement of damaged property.
(7) 
Statement of injuries sustained by Township employee or other party.
(8) 
Names and addresses of witnesses.
C. 
The report so filed shall not be for public information.
Employees shall not accept outside employment or engage in outside business activities without the prior written approval of the Township Administrator or other appointive authority. No application for permission to accept outside employment shall be granted unless such outside employment will not interfere with the employee's performance or compromise his position with the Township through a conflict of interest.
