The sketch plan initially submitted to the Board shall be based on an accurate base map at a scale (preferably not less than 100 feet to the inch) to enable the entire tract to be shown on one sheet. The sketch plan shall be submitted showing the following information:
A. 
The location of that portion which is to be subdivided in relation to the entire tract and the distance to the nearest existing street intersection.
B. 
All existing structures, wooded areas, marshlands, rock outcroppings and other significant physical features within the portion to be subdivided and within 200 feet thereof. Contours shall also be indicated at intervals of not more than five feet, extending 200 feet beyond boundary lines.
C. 
The name of the owner and of all adjoining property owners as disclosed by the most recent village and Town of Brookhaven assessment roles.
D. 
The Tax Map section, block and lot number, if available.
E. 
All the utilities available and all streets which are either proposed, mapped or built.
F. 
The proposed pattern of lots (including lot width, depth and approximate area), street layout, recreation areas, schematic indications of drainage, sewerage and water supply system [see § 159-19A(3) below] within the subdivided area.
G. 
All existing restrictions on the use of land, including easements, covenants or zoning lines.
A. 
In the case of minor subdivisions only, the subdivision plat application shall include all information shown on the approved sketch plan, plus the following additional information:
(1) 
A copy of such covenants or deed restrictions as are intended to cover all or part of the tract.
(2) 
An actual field survey of the boundary line of the tract, giving complete descriptive data by bearings and distances, made and certified to by a licensed New York State land surveyor. The corners of tract shall also be located on the ground and marked by monuments as approved by the Village Engineer and shall be referenced and shown on the plat.
(3) 
All on-site sanitation and water facilities shall be designed to meet the minimum specifications of the State and County Sanitary Code and the Public Health Law; and the endorsement of the Suffolk County Department of Health and Suffolk County Department of Environmental Control approving the designs shall appear on the plat, and such facilities shall comply with any applicable village ordinance, local law or regulation.
(4) 
Proposed subdivision name, Incorporated Village of Lake Grove, Town of Brookhaven and County of Suffolk.
(5) 
The date, North point, map scale, name and address of record owner and subdivider.
B. 
The plat to be filed with the County Clerk shall be printed on Mylar. The size of the sheets shall be as specified by the County Clerk.[1]

C. 
Design standards shall conform to those set forth in Article III.
The following documents shall be submitted for conditional approval:
A. 
Six copies of the preliminary layout (one on Mylar if tract is to be developed in sections), prepared at a scale of not more than 100 feet but preferably not less than 50 feet to the inch. The preliminary layout should be on sheets 24 inches by 26 inches, 24 inches by 36 inches or 36 inches by 40 inches. If more than one sheet is required, clearly indicated "match lines" shall be shown on all sheets, which shall be accompanied by a key map at such scale as to fit on one sheet only. The following items shall appear on the preliminary layout sheets:[1]
(1) 
The location of that portion which is to be subdivided in relation to the entire tract and the distance to the nearest existing street intersection.
(2) 
The names of all subdivisions immediately adjacent and the names of the owners of record of all adjacent property.
(3) 
Zoning district, including exact boundary lines of districts if more than one district, and any proposed changes in the zoning district lines and/or the text of Chapter 175, Zoning, applicable to the area to be subdivided.
(4) 
All parcels of land proposed to be dedicated to public use and the conditions of such dedication.
(5) 
Location of existing property lines, easements, buildings, rock formations, wooded areas, isolated single trees with a diameter of eight inches or more as measured three feet above the base of the trunk and other significant existing features for the proposed subdivision and adjacent property.
(6) 
Location of existing sewers, water mains, culverts and drains on the property, with pipe sizes, grades and direction of flow.
(7) 
Contours at intervals of five feet or less as required by the Board, including elevations on existing roads. Approximate grading plan if natural contours are to be changed more than two feet.
(8) 
The width and location of any street or public way or place shown on the Official Map or the Master Plan, if such exist, within the areas to be subdivided, and the width, location, grade and street profiles of all streets or public ways proposed by the developer.
(9) 
The approximate location and size of all proposed water lines, valves and sanitary sewer lines; connections to existing lines or alternate means of water supply or sewage disposal and treatment as provided in the State and County Sanitary Code, the Public Health Law and Suffolk County Department of Environmental Control. Profiles of all proposed water and sanitary sewer lines and fire hydrants shall be inspected and approved by the department having appropriate jurisdiction.
(10) 
A storm drainage plan indicating the approximate location and size of proposed lines and their profiles; connections to existing lines or alternate means of disposal.
(11) 
Plans indicating the proposed locations and type of streets, sidewalks, curbs, streetlighting standards, street trees, manholes and drain inlets; cross sections showing the width and depth of street pavements and subbase and the relative locations and cover on all underground conduits and utilities.
(12) 
Preliminary designs of any bridges or culverts which may be required.
(13) 
The number and approximate dimensions and area of each lot and the building setback lines.
(14) 
Where the topography is such as to make difficult the inclusion of any of the required facilities within the public area as laid out, the preliminary layout shall show the boundaries of proposed permanent easements over or under private property, which permanent easements shall not be less than 20 feet in width and shall provide satisfactory access to an existing public highway or other public open space shown on the subdivision map or the Official Map.
(15) 
An actual field survey of the boundary lines of the tract, giving complete descriptive data by bearings and distances, made and certified to by a licensed New York State engineer or land surveyor. The corners of tract shall also be located on the ground and marked by monuments of such size as approved by the Village Engineer and shall be referenced and shown on the plat.

B. 
A vicinity sketch indicating the proposed subdivision in relation to the surrounding area, at a scale no smaller than one inch equals 400 feet, shall accompany the preliminary layout submission. It shall show all existing subdivisions, streets, public buildings, schools, parks and playgrounds within 1/2 mile of the proposed subdivision.
C. 
If the application covers only a part of the subdivider's entire holding, a map of the entire tract, drawn at a scale of not less than 400 feet to the inch, showing an outline of the platted area with its proposed streets and an indication of the probable future street system with its grades and drainage in the remaining portion of the tract and the probable future drainage layout of the entire tract, shall be submitted. The part of the subdivider's holdings submitted shall be considered in the light of the entire holdings.
D. 
A copy of such deed covenants or restrictions as are intended to cover all or any part of the tract.
E. 
A clear plastic overlay sheet with street layout and lot lines, to be drawn to a scale of 400 feet to the inch.
The following documents shall be submitted for plat approval:
A. 
The plat shall be drawn on Mylar. The size of the sheets shall be 18 inches by 24 inches or 24 inches by 36 inches, including a margin for binding of two inches outside of the border along the left side and a margin of one inch outside of the border along the remaining sides. The plat shall be drawn at a scale of no more than 100 feet to the inch and oriented with North towards the top of the map where practicable. When more than one sheet is required, an additional index sheet of the same size shall be filed, showing to scale the entire subdivision with the lot and block numbers clearly legible. The plat shall show all information required for the preliminary layout plus the following additions and refinements:[1]
(1) 
Proposed subdivision name or identifying title and the Incorporated Village of Lake Grove, Town of Brookhaven and County of Suffolk, the name and address of record owner and subdivider and the name, license number and seal of the licensed surveyor.
(2) 
Streets, pedestrian ways, lots, reservations, easements and areas to be dedicated to public use, as well as large trees, groves and other natural features to be preserved.
(3) 
Sufficient data acceptable to the Village Engineer to determine readily the location, bearing and length of every street line, lot line, boundary line and to reproduce such lines upon the ground. Where practicable, these should be referenced to monuments previously established by a public authority.
(4) 
Permanent reference monuments shall be shown and shall be constructed in accordance with specifications of the Village Engineer. These monuments shall be set at all corners and angle points of the original tract to be subdivided, at street intersection corners, angle points in street lines, points of curve and tangency of street lines or any such locations as deemed necessary and appropriate by the Village Engineer.
(5) 
Lengths and bearings of all street lines, radii, length of curves, central angles, tangent distances for all street curves, measured to and along the center lines of such streets. All lengths shall be in feet and hundredths of a foot and all angles shall be given to the nearest 10 seconds, or closer if deemed necessary by the Village Engineer.
(6) 
Area of all lots in square feet and the dimensions and bearings of all lot lines. In the case of lots fronting on curved streets, sufficient information such as location of point of curvature and point of tangency shall be shown to enable a surveyor to establish the length and bearings of all boundary lines, a graphic scale and the true North point.
(7) 
Proposed street names, block, lot, section and house numbers in sequence.
(8) 
The plat shall also show by proper designation thereon all public open spaces for which deeds are submitted and those areas whose title is reserved by the developer.

B. 
Construction detail plans.
(1) 
Construction plans and profiles shall be submitted on convenient size prints, and every effort should be made to keep the various plans, sections and profiles a uniform size. The following construction details shall accompany the plat submission:
(a) 
Typical cross section and profiles of all proposed streets, showing existing and proposed grades as approved by the Village Engineer. The cross section shall show pavements and, where required, gutters, curbs and sidewalks. The materials and thickness of surfacing, base course and subbase shall be indicated.
(b) 
Profiles along the center lines of streets, showing existing and proposed elevations. Where a proposed new street intersects an existing street, the elevation of such existing street shall be shown for 100 feet from such intersection.
(c) 
Plans and profiles showing the exact location, size and invert elevations of existing and proposed water mains, sanitary sewers and stormwater drains, sewage pumping stations; profiles of all utility lines at points of interference; and the location of gas and other underground utilities or structures.
(d) 
Grading plan showing existing and proposed contours at two-foot intervals, referred to the same datum as Subsection B(1)(b) above. All elevations must be referred to established United States Government or approved local bench marks. The Board may require, where steep slopes exist, that present elevations of all proposed streets shall be shown at five points on a line at right angles to the center line of the streets every 100 feet throughout the length of such streets, and said elevation points shall be at the center line of the street, each property line and a point 30 feet inside of each property line.
(e) 
Detailed plans for bridges, culverts or similar structures.
(f) 
Locations of all proposed buildings, if required by the Board, and building setback lines.
(g) 
Upon consultation with the various private utility companies, the subdivider shall indicate on a plan the proposed type and location of the different utility lines (electric, gas, telephone, etc.).
(h) 
Planting plans showing the size, type and location of all street trees and an indication of the type and nature of planting treatment proposed in areas where eyesore conditions or erosion may occur.
(2) 
All designs for water lines, sanitary and storm sewers, sewage pumping stations, streets, bridge and drainage structures, etc., shall be prepared by a licensed professional engineer. The certification of the engineer shall appear on all construction plans.
C. 
All offers of deeds and other conveyances for streets, rights-of-way, easements and sites for public use, and copies of agreements, covenants or other documents showing the manner in which open space and areas to be reserved for the common use of the residents are to be maintained, all approved by the Village Attorney as to their legal sufficiency.
D. 
All water and sewerage facilities proposals contained in the subdivision plat shall be approved by the Suffolk County Department of Health and the Suffolk County Department of Environmental Control, whose endorsement shall appear on a print of the water supply and sanitary facility plans.
E. 
If the proposed subdivision will involve construction or activities which will have an influence or an effect on state, county or town right-of-way, the subdivider shall obtain letters from a responsible official of the appropriate Department of Public Works (state, county or town), addressed to the Chairman of the Board, indicating that agency's approval of the activity and that all necessary permits have or will be issued.
F. 
Letters directed to the Chairman of the Board and signed by a responsible official of the lighting agency, gas company, water company or any utility company or other authority which provides necessary utility service and has jurisdiction in the area, approving such proposed utility installation design, or a statement of intention by the utility that it will provide the necessary service facilities. A statement by the subdivider as to who will construct each facility, so that service will be available when required, shall also be submitted.
