All City employees witnessing any accident whereby any person sustains a personal injury or damage to property which may have been caused by any act of any officer or employee of the City or which may have been caused by the condition of any street, sidewalk or public way in the City or which may have been caused in or by the operation or use of any property, machinery, tools or implements belonging to the City shall, forthwith and within 48 hours thereafter, file with the head of the department in which they are employed a written report of the facts out of which said accident arose and describing the time when, the particular place where and the circumstances under which the damages or injuries were sustained, together with the names of all persons, with their places of residence, who witnessed the said accident.
The making and filing of such report shall not be construed as determining or tending to determine the liability of the City therefor.
All heads of departments shall, upon the receipt of a written report of an accident as aforesaid, cause a copy thereof to be forthwith filed in the office of the Corporation Counsel.
All heads of departments shall cause this chapter to be brought to the immediate attention of all City employees in their respective departments.
