Notice
Pursuant to Public Act 05-124, for property subject to a conservation or preservation restriction, and where activity is proposed within the restricted area, a notarized statement from the applicant certifying that:
the proposed activity involves only interior work in an existing building or exterior work that does not expand or alter the footprint of an existing building; or
the applicant provided written notice of such application, by certified mail, return receipt requested, not later than 60 days prior to the filing of the application to the party holding a conservation restriction or a preservation restriction; or
the holder of such restriction or the holder's authorized agent has submitted a letter verifying that the application is in compliance with the terms of the restriction.
7.
Building Drawings
Copies of all plans submitted to the Building Official in connection with this project.
8.
Plot Plan
A plot plan or site plan at acceptable scale incorporating an A-2 survey of the premises or parcel of land prepared, stamped with an embossed seal and signed by a professional land surveyor showing, where applicable:
8.1.
the actual dimensions of the plot to be built upon or used;
8.2.
the location of the plot in relation to public and/or private streets and accessways;
8.3.
the zone in which the plot is located;
8.4.
the north arrow;
8.5.
the location and size of any presently existing buildings, signs, and other structures upon the plot with exact setback distances from front, side and rear lot lines;
8.6.
the location of septic system(s) and well(s);
8.7.
the proposed location and size of any new buildings, additions, signs, or structures to be erected thereon, with proposed setback distances from front, side and rear lot lines;
8.8.
current and proposed percentage of building coverage;
8.9.
current and proposed percentage of impervious coverage;
8.10.
current and proposed total disturbed area;
8.11.
current and proposed building height;
8.12.
all applicable setback lines;
8.13.
a signed statement from a professional architect or surveyor stating the existing and proposed floor area (where applicable);
8.14.
the location of any wetlands and/or watercourses as delineated by professional soils scientist;
8.15.
the location of any flood hazard zones as delineated by the Flood Insurance Rate Map and published by FEMA;
8.16.
the location of any conservation or preservation restrictions;
8.17.
the location of any public drinking water supply areas;

a key map showing the location of the property in relation to surrounding areas;
8.19.
title block with name of project, name of developer, address, date, and scale;
8.20.
proposed location and dimensions of all exterior signs; (for sign applications/changes in signage);

parking plan showing number and location of existing parking spaces, demonstrating how needs will be met (for proposed change in use); and
8.22.
the Zoning Enforcement Officer (ZEO) may waive requirements determined to not be necessary for the review of such zoning permit.
9.
Erosion
Two copies of an Erosion and Sedimentation Control Plan in compliance with the Connecticut Department of Environmental Protection 2002 Guidelines for Soil Erosion and Sediment Control (CTDEP Bulletin 34) for all new construction, and for any other application where determined necessary by the ZEO.
10.
Wetlands
Copy of Inland Wetlands and Watercourses Agency (IWWA) or Authorized Agent approval if the application involves any activity regulated pursuant to Connecticut General Statues (CGS) §§ 22a-36 to 22a-45.
11.
Aquifer
Copy of Aquifer Protection Agency (APA) approval if the application involves any activity regulated pursuant to CGS §§ 22a-336 to 22a-400.
12.
Flood
An elevation certificate from an authorized professional if proposal is within a FEMA designated flood zone (only applies to new construction or substantial improvements) OR a copy of Zoning Commission Approval (acting as the Flood Plain Protection Board) certifying the proposed activity complies with § 470-602.
13.
Conditions
Copy of Commission conditions - Any maps, plans, documents, statements, and stipulations submitted to and approved by the Commission, in connection with any conditions of approval imposed by the Commission, shall be conditions for approval of a zoning permit, whether or not stated on the zoning permit approval.
14.
Conditions
Copy of other conditions - Any maps, plans, documents, statements, and stipulations submitted to and approved by the Commission, IWWA, ZBA, WPCA or other Town agency, in connection with any conditions of approval imposed by such Commission, Board or Agency shall be conditions for approval of a zoning permit, whether or not stated on the zoning permit approval.
15.
Compliance with Conditions
Any conditions of approval imposed by the Commission required to be carried out prior to the commencement of any site activities or issuance of a building permit.
16.
Bonds
Any required bonds submitted to the Town of Canton Finance Officer and Town Attorney for review and determined to be acceptable.
17.
Filings
Any required certificates of action filed on the land records.
18.
Well/Septic
Copy of approvals of the Farmington Valley Health District (FVHD) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of a private septic system or well.
19.
Public Water
Copy of approvals of the Connecticut Water Company (CWC) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of a public water supply system.
20.
Sewer
Copy of approvals of the Town of Canton Water Pollution Control Authority (WPCA) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of public sewage disposal system.
21.
Historic
Copy of certificate of appropriateness, if located within the Collinsville Historic District or Canton Center Historic District.
22.
Work in Town ROW
Copy of approval for work within the Town right-of-way (ROW) and/or driveway permit, if required.
23.
Traffic
Comments from the Local Traffic Authority, if required.
Copy of approval of the Office of the State Traffic Administration, if required.
24.
Markings
The placement of stakes or markers or flagging on the lot by a professional land surveyor or engineer indicating the following [If required by the ZEO (for new construction)]:
i.
the location of proposed building construction and lot lines; and
ii.
all "limits of clearing" on the lot (consistent with that shown on any approved site plan) for construction; utility installation; access ways; parking areas; staging and stockpiling of materials; and, any other proposed activities.
25.
Measurements
Measurements, dimensions, and calculations of any construction features subject to the requirements of these regulations, including but not limited to, setback distances (if deemed necessary by the ZEO to determine compliance with these regulations). The ZEO may require such measurements, dimensions, and calculations to be prepared by a professional land surveyor.
26.
Height
Dimensioned elevation/renderings depicting the height of all proposed structures above the proposed finished grade.
27.
Signs
Dimensioned elevation/renderings of all proposed signs demonstrating compliance with the standards of § 470-703.
28.
Existing Conditions
Photographs of existing conditions (unless new building).
29.
Accessory Dwellings
If an accessory dwelling unit, an affidavit indicating that the owner of the property will occupy either the principal or accessory dwelling unit and will comply with the standards of § 470-303C(1).
30.
Home Based Business
If a home based business, an affidavit indicating that the owner of the property that the home based business will be located within the principal dwelling, or on the premises, and operated by a resident owner of the premises and will comply with the standards of § 470-303C(3).
31.
Inspection
The site has been inspected as authorized by § 470-908B(6) of the regulations.
32.
Notice
The applicant is informed that by receiving the zoning permit that such applicant may provide notice of such permit by either publication in a newspaper having substantial circulation in such municipality stating that the permit has been issued or any other method provided for by ordinance.
1.
Application Form
A completed zoning permit application form written statement describing the proposed work and the purpose of the excavation and/or filling with an indication of proposed commencement and completion dates.
2.
Application Fee
Fee required by Chapter 248 of the Town of Canton Town Code.
3.
Property Identification
Copy of the Assessors card and property deed.
4.
Abutters
Names of surrounding property owners, surrounding street and proposed access.
5.
Narrative
A written statement describing the proposed work and the purpose of the excavation and/or filling with an indication of proposed commencement and completion dates.
6.
Calculations
A written statement calculating the number of cubic yards of material to be imported and/or exported, and indicating the hours and days of the week for the operation; and the number, kind, and size of vehicles, number of trips by each vehicle, and other equipment to be used.
7.
Plan
A mapped plan of the property to be excavated or filled showing:
existing and proposed contour lines with existing contours shown for at least 20 feet beyond the perimeter property lines;
lot lines and streets adjoining the lot and the names of owners of each abutting the lot;
the location and exterior limits of the area to be excavated, graded or filled;
the location of any areas for the staging or storage of material;
the location of watercourses, wetlands soils within 200 feet of the proposed earthwork;
the location of flood hazard areas;
the location on the lot of any wooded areas, rock outcrops and existing and proposed buildings, structures and processing equipment (excluding earth material processing, screening or crushing equipment (allowed by special permit in an Industrial District); and
proposed vehicular access to and circulation within the lot.
8.
Drainage
Existing and proposed drainage plans.
9.
Wetlands
Copy of Inland Wetlands and Watercourses Agency (IWWA) or Authorized Agent approval if the application involves any activity regulated pursuant to Connecticut General Statues (CGS) §§ 22a-36 to 22a-45.
10.
Flood
An elevation certificate from an authorized professional if proposal is within a FEMA designated flood zone (only applies to new construction or substantial improvements) OR a copy of Zoning Commission approval (acting as the Flood Plain Protection Board) certifying the proposed activity complies with § 470-602.
11.
Erosion
Plans, specifications, or other information necessary to describe the erosion and sedimentation control measures to be utilized before, during, and after any proposed activities
12.
Erosion
A statement that any such activity will be conducted in accordance with § 470-706 (Erosion and Sediment Control) of these regulations.
13.
Bonds
Any required bonds or security submitted to the Town of Canton Finance Officer and Town Attorney for review and determined to be acceptable.
14.
Commission Conditions
Copy of Commission Conditions - Any maps, plans, documents, statements, and stipulations submitted to and approved by the, IWWA, Zoning Commission, or Planning Commission, in connection with any conditions of approval imposed by such Commissions, if the proposed earthwork is part of a previously approved subdivision or Zoning approval.

Copies
Unless otherwise specified by herein, each application shall be submitted with 11 copies of the following information.
2.
Application Form
A completed application form, as adopted by the Commission, bearing the original signature(s) of the owner(s) of record, or letter of authorization from the owner to a designated agent.
3.
Application Fee
The complete application fee, as required by Chapter 248 of the Town Code. This may include additional review fees for technical expertise, engineering, and other reviews if required by the Commission.
4.
Narrative
A written statement, signed by the applicant, and by the owner if different from the applicant, describing the following in sufficient detail to determine compliance with these regulations and to establish the nature and extent of the proposal; 14 copies shall be submitted;
1.
a detailed narrative description as to the nature and extent of the proposed use or occupancy;
2.
provision to be made for water supply, sewage disposal, solid and liquid waste disposal, drainage and other utilities;
3.
the number of persons estimated to occupy or visit the premises on a daily basis, and the basis for determining the parking and loading requirements for the use; provisions for pedestrian and bicycle access within and into the site, where appropriate;
4.
an estimate of the type of vehicular traffic and number of vehicle trips to be generated on a daily basis and at peak hours;
5.
the equipment or other methods to be established to comply with required performance standards; and
6.
disclosure of any toxic or hazardous materials to be used, stored or processed in connection with the proposed use or occupancy as identified in the U.S. Environmental Protection Agency list of priority pollutants, Section 3001 of the Resource Conservation and Recovery Act (40 CFR Part 261) or the State of Connecticut Hazardous Waste Regulations, which disclosure shall include a description of how such materials are to be managed and a report on the status of permits and approvals required from Federal, State and Town agencies having jurisdiction.
5.
Deed and Easements
Copies of draft deeds or easements for any road widening, access, driveways, drainage, conservation, utilities or other easements, restriction or covenants proposed.
6.
Notice
For new construction or other activity considered to be significant in the sole judgment of the Commission, the Commission may hold a public hearing on the application; and require that the applicant:
1.
post a sign owners in accordance with the requirements of § 470-909F of the Zoning Regulations; and
2.
give notice to abutting property owners in accordance with the requirements of § 470-909G of the Zoning Regulations.
7.
Notice
Notification to parties holding restrictions may be required in accordance with the requirements of § 470-909K.
Pursuant to CGS § 47-42d, for property subject to a conservation or preservation restriction, and where activity is proposed within the restricted area, a notarized statement from the applicant certifying that:
1.
the proposed activity involves only work that does not expand or alter the footprint of an existing building, or
2.
the applicant provided written notice of such application, by certified mail, return receipt requested, not later than 60 days prior to the filing of the application to the party holding a conservation restriction or a preservation restriction, and the holder of such restriction or the holder's authorized agent has submitted a letter verifying that the application is in compliance with the terms of the restriction.
8.
Notice
Notification to adjoining municipalities may be required in accordance with the requirements of § 470-909H.
9.
Notice
Notification to water companies may be required in accordance with the requirements of § 470-909J.
10.
Wetlands
If a site plan application involves an activity regulated pursuant to CGS §§ 22a-36 to 22a-45, inclusive, the applicant shall submit an application for a permit to the Inland Wetlands and Watercourses Agency not later than the day such application is filed with the Commission.
11.
Wetlands
Where applicable, a soils report from a professional soil scientist.
12.
Wetlands
Where applicable, a statement from a professional soil scientist that the "flagging" of wetland soils and delineation of watercourses on the maps is accurate.
13.
Aquifer
Copy of Aquifer Protection Agency (APA) registration or permit approval if the application involves any activity regulated pursuant to CGS §§ 22a-336 to 22a-400.
14.
Flood
An elevation certificate from an authorized professional if proposal is within a FEMA designated flood zone (only applies to New Construction or Substantial Improvements).
A copy of Zoning Commission approval (acting as the Flood Plain Protection Board) certifying the proposed activity complies with § 470-602.
A copy of a completed Checklist 1.6 for additional requirements in flood-prone areas.
15.
Well/Septic
Copy of approvals of the Farmington Valley Health District (FVHD) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of a private septic system or well.
16.
Public Water
Copy of approvals of the Connecticut Water Company (CWC) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of a public water supply system.
17.
Sewer
Copy of approvals of the Town of Canton Water Pollution Control Authority (WPCA) for any proposed or change in use, or proposed building or structure that involves the installation, extension, relocation, reconstruction, or connection of public sewage disposal system.
18.
State Traffic Administration and Local Traffic Authority
Copy of application review comments from the Canton Local Traffic Authority.
Copy of application materials to the Office of the State Traffic Administration, if required.
19.
Other Conditions
Copy of other conditions - Any maps, plans, documents, statements, and stipulations submitted to and approved by the Commission, Planning Commission, IWWA, ZBA, WPCA or other Town agency, in connection with any conditions of approval imposed by such Commission, Board or Agency shall be conditions for approval of a zoning permit, whether or not stated on the zoning permit approval.
20.
Bonds
Any proposed bonds or other security estimates submitted to the Commission's Engineer for review and determination of adequacy.
21.
Historic District
Copy of certificate of appropriateness, if located within the Collinsville Historic District or Canton Center Historic District.
22.
Stormwater
Three copies of a stormwater management study, if required, and/or written certification by a professional engineer that the proposed stormwater management system has been designed in accordance with the Connecticut Department of Energy and Environmental Protection (DEEP) 2004 Stormwater Quality Manual, Best Management Practices specified by the DEEP, and complies with the standards of § 470-713.
23.
Traffic
Three copies of a Traffic Study and other documentation as required by § 470-708.
24.
Erosion Control
Written certification by a professional engineer that the Erosion and Sedimentation Control Plan has been designed in accordance with the DEEP 2002 Guidelines for Soil Erosion and Sediment Control (CTDEP Bulletin 34).
A completed Checklist 1.4. Requirements for erosion and sediment control.
A narrative describing construction stages and manner in which the application complies with the requirements for erosion and sedimentation control prescribed under these regulations.
25.
Site Plan
A site plan shall be prepared in accordance with these regulations and standards, and showing the information hereinafter required.
25.1.
Preparation - The site plan shall be clearly and legibly drawn on good quality fixed line mylar or other material that will enable production of clear prints as required by these regulations. The site plan shall be prepared by and shall bear the name, signature, and seal of a professional: surveyor, engineer, architect; and/or landscape architect, each as required by law for preparation of parts of the site plan. The seal shall be impressed on all copies of the site plan presented for approval.
25.2.
Size, Scale, and Copies
The site plan shall be drawn to a scale of not less than one inch equals 40 feet or such greater scale as may be necessary to show the site development features clearly and in detail. The parts of the site plan that are intended for approval will be required to be endorsed and filed in the office of the Town of Canton Town Clerk

Plan Information - The information listed herein is the minimum required to be shown on the site plan to the extent that the information occurs on or is applicable to the particular lot or is required to demonstrate compliance with these regulations.
25.4.
General
A key map at a scale if 1"=500' showing the location of the property in relation to surrounding areas, including streets, property lines and zoning district boundaries, and an index map if the plan is divided into sections or phases, or is of such size that more than one sheet for each plan section is required.
25.5.
General
Title block with name of project, name of developer applicant and owner, address, date (with revisions), and scale.
25.6.
General
North arrow, numerical, and graphic scale (the title of individual site plan sheets and orientation of north arrows shall, to the extent practicable, be consistent from one sheet to the other).
25.7.
Zoning Schedule
A schedule, certified by a professional surveyor or engineer, specifying in square feet the:
1.
lot area;
2.
yard setbacks;
3.
area of wetlands and watercourses, as determined by a professional soils scientist;
4.
amount of floor area by use;
5.
building coverage;

impervious coverage;
7.
maximum height;
8.
calculations for required parking and loading;
9.
calculations for required landscape areas;

total disturbed area;

total cuts; and

25.8.
Boundaries
Boundaries of and existing conditions of the lot based on a "Class A-2" type survey as defined in the Regulations of State Agencies adopted pursuant to CGS § 20-00b.
25.9.
Abutters
Names of all abutting lot owners as recorded with the Town of Canton Assessor.
25.10.
Limits
Any line delimiting a portion of the lot to be used under the application and any zoning district boundary on the lot.

25.31.

identification of building materials;
3.
textures and color of all building facades;
4.
fenestration;
5.
roofs;
6.
building illumination;
7.
rooftop units that will project above the level of a parapet or other enclosure;
8.
rooftop enclosures;
9.
outdoor display areas, merchandise, vending machines or items along a building that would obscure the building facade; and
10.
any other appurtenances.
26.2.
Floor plans showing:
1.
existing and proposed floor plans with dimensions; and
2.
identification of all proposed interior and exterior alterations, modifications or changes.
27.
Computer Simulation
When warranted by the size, location or nature of a proposal, or when determined to be in the public interest, the Commission may require a three dimensional physical representation or a computer simulation of the proposal at an appropriate scale. The Commission may also require that the model include three dimensional representation of all or portions of the abutting lots if this would significantly aid the Commission and the public to visualize and understand the proposal.

Additional Information
The Commission may, in accordance with the requirements of these regulations require the submission of additional information as deemed necessary to make a reasonable review of the application.
30.
Digital Submission
In addition to the submission requirements above, the applicant shall submit in digital format (PDF or JPEG, as deemed appropriate by Land Use staff) all application materials and public hearing exhibits, or, in the absence of a public hearing, any supplemental information received up to final action by the Commission. These materials shall include, but not be limited to, the application form, cover letter, statements of use, site plan and architectural plans, reports, easements or deeds to roads, and any other information submitted to support an application. The Commission may waive this requirement upon request by the applicant showing good cause.
31.
Waiver
The Commission, per § 470-901A of the regulations may waive the required submission of all or a part of the required information after finding that such information is not necessary in order to render a decision on the application or defer the required submission of part of such information to a later date.

Copies of a soil erosion and sediment control plan containing proper provisions to adequately control accelerated erosion and sediment and reduce the danger from stormwater runoff on the proposed site based on the best available technology.

Copies of a narrative describing containing the information detailed in the "2002 Guidelines for Soil Erosion and Sediment Control," as may be amended, in addition to the following:
2.1.
The development;
2.2.
The schedule for grading and construction activities including:
start and completion dates;
sequence of grading and construction activities;
sequence for installation and/or application of soil erosion and sediment control measures;
sequence for final stabilization of the project site;
2.3.
The design criteria for proposed soil erosion and sediment control measures and stormwater management facilities;
2.4.
The construction details for proposed soil erosion and sediment control measures and stormwater management facilities;
2.5.
The installation and/or application procedure for proposed soil erosion and sediment control measures and stormwater management facilities;
2.6.
The operations and maintenance program for proposed soil erosion and sediment control measures and stormwater management facilities;
2.7.
An estimate of the cost to implement and maintain the soil erosion and sediment control plan over the estimated life of the project.
Itemized costs estimates by a professional engineer for site improvements and the installation of all erosion and sediment controls based on published Connecticut DOT unit prices.
3.
Erosion and Sediment Control Plan Checklist
Copies of a plan drawn to an appropriate scale containing the information detailed in the checklist in Section 3.1 (page 3-12) of said 2002 Guidelines, as may be amended, in addition to the following:
3.1.
The location of the proposed development site, its boundaries, its size, and adjacent properties;
3.2.
The existing and proposed topography shown with contour lines at intervals not greater than two feet and extending for a distance of 200 feet beyond the boundaries of the development site;
3.3.
The boundaries and location of various soil types, wetlands, watercourses and water bodies and other land subject to periodic flooding on the site and within a distance of 200 feet beyond the site boundaries, as delineated by a professional soils scientist;
3.4.
The location and size of any existing and proposed structures, drainage facilities, roadways and other man-made installations on the land as well as drainage structures outside the boundaries for a distance of 200 feet;
3.5.
The location of and design details for all proposed temporary and permanent erosion and sediment control measures and stormwater management facilities;
3.6.
The sequence of grading and construction activities; the sequence for the installation and/or application of soil erosion and sediment control measures; and the sequence for final stabilization of the development site;
3.7.
Engineer Seal - Such plans, computations, design criteria, and other technical information submitted to the Commission shall be signed and sealed by a professional engineer;
3.8.
Surveyor Seal - Topography, perimeter survey, and other land boundary information submitted to the Commission shall in accordance with Standards as prescribed in the "Code of Recommended Practice for Standards of Accuracy of Surveys and Maps," Oct. 1976, as may be amended and shall be signed and sealed by a professional Land Surveyor.
4.

Any other information which in the Town Planners judgment will assist in evaluating the proposal.

Pursuant to CGS § 47-42d, for property subject to a conservation or preservation restriction, and where activity is proposed within the restricted area, a notarized statement from the applicant certifying that:
1.
the proposed activity involves only work that does not expand or alter the footprint of an existing building; or
2.
the applicant provided written notice of such application, by certified mail, return receipt requested, not later than 60 days prior to the filing of the application to the party holding a conservation restriction or a preservation restriction, and the holder of such restriction or the holder's authorized agent has submitted a letter verifying that the application is in compliance with the terms of the restriction.

Notification to adjoining municipalities may be required in accordance with the requirements of § 470-909H.

Uses Proposed
A statement describing in detail the proposed use or uses.

13.
Traffic
For any proposed nonresidential development containing a gross floor area of 10,000 square feet or more, any intersection that would have 100 or more vehicles trips in a peak hour, or for any development which, in the Commission's judgment, could generate high levels of traffic, exacerbate existing traffic conditions, or create a traffic safety issue, the applicant shall provide a traffic impact analysis, in accordance with the Institute Of Transportation Engineers requirements for traffic impact studies. Such analysis shall evaluate traffic generated by a proposed development and its probable impact on existing roads and intersections in the area in accordance with § 470-708B and shall include the following:
13.1.
existing and projected traffic volumes (average daily traffic, peak a.m. and p.m.) and peak hour traffic generation;
13.2.
past and present roadway conditions including location and number of accidents;
13.3.
existing roadway capacity; volume and capacity ratios;
13.4.
proposed sight lines;
13.5.
location of existing and proposed curb cuts, traffic lights and intersections at the development site and within 300 feet from the development site;
13.6.
traffic impact of proposed development, including but not limited to, the effect of the proposed development on traffic conditions on abutting streets and any nearby intersections that would have 100 or more vehicle trips in a peak hour;
13.7.
the patterns of vehicular circulation in relation to the adjoining street system;
13.8.
adequacy of:
1.
right-of-way and travel way;
2.
traffic signalization, traffic channelization, left-turn lanes and roadway widths of adjoining streets;
3.
vehicular stacking lanes and/or distances;
4.
pedestrian drop-off areas; and
5.
other traffic or transportation facilities to accommodate the proposed development.
13.9.
recommendations for safe pedestrian, bicycle, and vehicular circulation.
13.10.
where applicable, the applicant shall include the written recommendations of the Connecticut Department of Transportation, the Canton Traffic Authority, and the Commission's engineer.
13.11.
whether a significant traffic impact will occur as identified under § 470-708B(4).
14.
If a multifamily development, 15 copies of a report of the proposal including:
14.1.
Number of units proposed and breakdown of dwelling unit types;
14.2.
The density of the proposed development (units per gross acre);
14.3.
Acreage in buildings and parking and acreage in open space;
14.4.
Anticipated number of people and number of school age children per unit;
14.5.
Projected dwelling unit floor areas;
14.6.
A draft of proposed covenants and restrictions;
15.
Traffic
Comments from the Local Traffic Authority.
Copy of approval of the Office of the State Traffic Administration.
16.
Staging
A statement describing the proposed staging if the development is to be constructed over a period of years.
17.
Fire
A report from the Canton Fire Marshal commenting and/or recommending on fire protection provisions affecting the development or nearby properties.
18.
Topo/Drainage
In cases where unusual topographic, drainage or other conditions exist, the Commission may require the submission of additional data pertinent to their review.
19.
Construction Schedule
A narrative describing construction stages and manner in which the application complies with the requirements for erosion and sedimentation control prescribed under these regulations.
20.
Special Permit Criteria
A narrative describing compliance with the special permit criteria of § 470-902E.
21.
Additional Information
Any other information which in the Commission's judgment will assist in evaluating the proposal.
1.
Fifteen copies of maps or plans at an appropriate scale showing:
1.1.
base flood elevation data and floodway data available from a federal, state, or other source;
1.2.
the location of any water courses, flood boundaries, and flood hazard areas delineated by the Flood Insurance Rate Map and published by FEMA;
1.3.
the location of existing and proposed structures, and drainage facilities.
1.4.
the actual elevation of the lowest habitable floor (including basement) of all new or substantially improved structures. Such elevation shall be the North American Vertical Datum of 1988 (NAVD 1988) to which all elevations associated herewith, including BFEs, are, or are to be, referenced.
1.5.
elevation to which any structure has been floodproofed;
1.6.
description of the extent to which any watercourse will be altered or relocated as a result of a proposed development;
2.
As necessary, 15 copies of:
2.1.
Certification by a professional land surveyor of the actual elevation of all existing and proposed structures.
2.2.
Certification by a professional engineer or architect of the flood proofing methods for any nonresidential structure;
2.3.
Plans to enclose space below the base flood level; and
2.4.
A statement as to whether there will be dry access to the structure during the 100-year storm event
2.5.
A statement as to whether the proposed development will increase the water surface elevation of the base flood more than one foot at any point after considering the cumulative effect of the proposed development when combined with other anticipated development.
3.
A statement that all necessary permits have been obtained from federal, state, or local governmental agencies from which prior approval is required.
4.
A statement regarding whether the alteration or relocation of a watercourse is proposed in a form acceptable for notification of adjacent communities, the Connecticut Department of Energy and Environmental Protection, and the Federal Insurance Administrator
5.
A statement indicating whether maintenance will be provided within the altered or relocated portion of said watercourse so that the flood carrying capacity will not be diminished.
6.
A statement indicating whether interpretations were made as to the exact location of the boundaries of the areas of special flood hazards (i.e., where there appears to be a conflict between a mapped boundary and actual field conditions).
7.
§ 470-602, Floodplain.
Location, design, and details of all existing and proposed activity within flood hazard areas, calculations and other information necessary to demonstrate compliance with § 470-602.
8.
Any other information which in the Commission's judgment will assist in evaluating the proposal.

6.
Schedule
A schedule, certified by a professional surveyor or engineer, specifying in square feet the:
1.
lot area;
2.
yard setbacks;
3.
area of wetlands and watercourses, as determined by a professional soils scientist;
4.
total disturbed area;
5.
total cuts; and
6.
total fills.
7.
Plan
Copies of a plan, drawn to an appropriate scale, showing:
7.1.
A survey prepared by a professional surveyor identifying spot elevations and contours at intervals not greater than two feet extending for a distance of 200 feet beyond the boundaries of the site;
7.2.
Location and extent of watercourses, wetlands and boundaries of land subject to periodic flooding on the site and for a distance of 200 feet beyond the boundaries of the site; and
7.3.
The soil types and their location as identified in the soil survey of the Town of Canton prepared by the U.S. Soil Conservation Service;
7.4.
Lots and streets within 500 feet;
7.5.
Lot lines and streets adjoining the lot and the names of owners of each abutting the lot;
7.6.
The location and exterior limits of the area to be excavated, graded or filled;
7.7.
Existing and proposed contour lines on the parcel, drawn to a scale of not less than 40 feet to the inch and with a contour interval not exceeding two feet;
7.8.
The location of any areas for the staging or storage of material;
7.9.
Existing and proposed drainage on the lot and existing watercourses, water bodies, and wetlands on or within 200 feet of the parcel;
7.10.
Proposed vehicular access to and circulation within the lot;
7.11.
The location on the lot of any wooded areas, rock outcrops and existing and proposed buildings, structures and processing equipment;
7.12.
Exiting or proposed site drainage;
7.13.
Existing groundwater table.
8.
Report
An engineering report including statements on traffic safety, noise, grading, landscaping, and erosion control methods;
9.
Markings
A surveyor's statement certifying that the areas planned for immediate and future excavation have been staked out in a permanent manner visible to operators during excavation;
10.
Timeframe/Schedule
An engineering plan detailing, over a period of time not to exceed two years, the manner in which the site owner/operator intends to progress in furthering site excavation and extraction of earth material.
11.
Erosion
An Erosion and Sedimentation Control Plan in compliance with the Connecticut Department of Environmental Protection 2002 Guidelines for Soil Erosion and Sediment Control (CTDEP Bulletin 34) for all new construction showing the location, design, dimensions, and details of all existing and proposed erosion and sedimentation controls necessary to demonstrate compliance with § 470-706 and the Requirements for Erosion and Sediment Control Checklist.
12.
Ponding
If a pond may result due to excavation below normal water table, the application shall also include:
12.1.
A drainage analysis showing watershed area;
12.2.
Computations of water inflows and outflow; and
12.3.
Calculations and confirmation that the pond's water supply and its water inflow and outflow will be adequate to avoid stagnation and will not be hazardous to surrounding land uses.
13.
Processing
Detailed plans, specifications and other information necessary to describe any earth material processing, screening and rock crushing.
14.
Re-use
A re-use plan establishing that upon reaching finished grades, the site will be suitable for development of a use that is permitted in the subject zone.
15.
Costs
Itemized costs estimates by a professional engineer for all site improvements and the installation of all erosion and sediment controls based on published Connecticut DOT unit prices.
16.
Additional Information
Any other information which in the Commission's judgment will assist in evaluating the proposal.
17.
Additional Considerations
In cases of excavation, filling, removal, dredging earth material processing, rock crushing, screening or other grading within the limits of the Floodplain District, the Commission may further consider:
17.1.
Any approval of the Connecticut Department of Environmental Protection and/or the U.S. Army Corps of Engineers;
17.2.
Possible deleterious effects of the proposed work in regard to flood or pollution, either before or after completion; and
17.3.
Lateral support slopes, grades and elevations of abutting streets and properties.

16.3.
Properties within 500 feet in all directions of the premises proposed to be rezoned, and the names and addresses of the owners of such parcels as indicated on the most recent Grand List. In the event that a petition is received in accordance with § 470-904C(5), the applicant shall submit the square footage of land located within 500 feet of the subject zone change parcel for each parcel owner of record.
