A. 
The Board of Supervisors of the Township of Hellam shall and hereby is empowered to establish a form document entitled "Notice of Ordinance Violation," which may be amended from time to time at the discretion of the Board of Supervisors. Such civil enforcement notice shall contain such information as deemed appropriate by the Board of Supervisors, in its discretion, but at a minimum, the following information:
(1) 
The name, address and telephone number of the violator;
(2) 
The location, date and time of the violation;
(3) 
A citation of the ordinance violated and a general description of the nature of the offense;
(4) 
The amount of the civil fine to be paid within 15 days of the notice of ordinance violation, the date by which it must be paid, and the amount of the fine in the event civil enforcement proceedings are brought, and a judgment entered, by the appropriate Magisterial District Judge;
(5) 
A notice stating that the commencement of a civil action before the appropriate Magisterial District Judge will result in the imposition of filing fees, other court costs, and reasonable attorney's fees incurred by the municipality;
(6) 
The name and the address of the entity to whom the fine is to be paid;
(7) 
A signature and title of the officer issuing the notice.
B. 
Service of the Notice of Ordinance Violation shall be affected by the enforcement officer hand delivering a copy of the notice to the violator, or by a Hellam Township police officer, and/or by mailing of same to the violator by certified mail, return receipt requested.
A. 
The following persons shall be authorized to issue civil enforcement notices and execute and file civil complaints before the appropriate Magisterial District Judge, for the enforcement of Township Ordinance remedies and penalties:
(1) 
Police officers for Hellam Township;
(2) 
Hellam Township Code Enforcement Officer; and
(3) 
Hellam Township Manager.
B. 
In addition, staff employees of the Township may be designated, from time to time, by resolution of the Board of Supervisors to perform such functions.
