The Police Department is hereby authorized to furnish application forms to any person desiring to have issued a personal identification card, which application form is to be signed by the applicant and to remain as a permanent record of the Police Department.
A fee of $3 shall be charged for the furnishing of a personal identification card, which charge will include the cost of furnishing a photo of the applicant. No identification card will be issued unless an application is duly completed and signed by the person requesting said identification card.
Once an application has been signed and filed with the Police Department, the applicant may obtain additional identification cards upon the payment of a fee of $3 without the necessity of filing a further application.

All fees collected by the Police Department pursuant to this section shall be deposited with the Director of Finance.
