Every application for a permit under this chapter
shall include a properly prepared long-form environmental assessment
form (EAF), which shall be reviewed by the City Engineer and referred
by him, if appropriate, to the City Environmental Quality Review Board
and/or site plan review agency for further review and recommendation.
