To obtain a copy of criminal history record
information maintained by the Butler Township Police Department, you
must:
A.
Complete Form (SP 4-164), Request for Criminal History
Record Information, in duplicate, according to instructions.
B.
Attach a check or money order in the amount in an
amount as set forth in the schedule of fees, on file in the Township
Manager's office, payable to the "Butler Township Police Department"
when the request is made by a non-criminal-justice agency or individual.
