Effective July 1, 2012, an employee of the Borough of South Plainfield ("Borough") who is eligible for other health-care coverage may waive the health-care coverage under the Borough's health benefits program pursuant to N.J.S.A. 40A:10-17.1.
In order to be eligible for this benefit, an employee must complete a new Change in Coverage or Termination of Benefits form and submit same along with proof of alternative health coverage (if waiving health coverage) to the Borough Administrator's office. Likewise, to reinstate coverage, this same form may be used. In each case, the form must be certified by the Borough Administrator in order for the request to be effective.
In consideration of the filed waiver, the Borough will pay, semiannually to the eligible employee, benefits as set forth in the attached policy entitled "Borough of South Plainfield, Waiver of Health Benefit Coverage."[1]

Any employee who waives coverage shall be permitted to resume coverage under the same terms and conditions as apply to initial coverage if the employee ceases to be covered through the employee's spouse's benefit plan for any reason. An employee who resumes coverage may be required to repay a pro rata portion of any previously paid lump sum that represents an advance payment for any period of time for which the waiver has been revoked. An employee wishing to resume coverage shall complete, sign and file with the Borough Administrator's office an official form for reinstatement.
